It develops when oil and dead skin cells accumulate inside an open pore. Over time, the material can become hardened and the pore may become noticeably enlarged.

Several factors can contribute to blackheads, including excess oil production, hormonal changes, genetics, and the buildup of dead skin cells. Some unusual or clustered lesions can also be associated with conditions such as Favre-Racouchot syndrome, which is often linked to long-term sun exposure and commonly affects the skin around the eyes and temples.

Although squeezing a blackhead may be tempting, trying to dig out a deep or unusually large one at home can cause bleeding, inflammation, infection, and permanent scarring. A dermatologist can examine the area and, when appropriate, perform a professional extraction. Treatments such as salicylic acid, topical retinoids, or chemical peels may also be recommended depending on the individual’s skin and the underlying cause.

Not every dark spot or enlarged pore is actually a blackhead. Sebaceous filaments, for example, are normal structures that help move oil toward the skin’s surface. They can look similar to blackheads but are a natural part of the skin and should not be aggressively removed.

If a dark or enlarged spot is unusually large, painful, swollen, repeatedly returns, or does not improve with ordinary skincare, it is worth having a dermatologist examine it. What appears to be an extreme blackhead may have another explanation, and professional evaluation can help determine the safest treatment while reducing the risk of lasting skin damage.

WATCH VIDEO:

An unusually large blackhead, sometimes called a giant comedo, can look very different from the small blackheads most people occasionally notice.
